Authorization for Release of Information If a member’s disability insurance claim and their appeal to the insurer is rejected the member’s claim can be referred to the Disability Insurance Plan Board of Management. To make this referral an Authorization for Release of Information form needs to be filled.
Employment Insurance Sickness Benefits. Members may qualify for Employment Insurance (EI) sickness benefits during their elimination period while they are on leave without pay and waiting for their disability insurance to begin.
