Release Form

I, being competent and of legal age, hereby give Public Service Alliance of Canada the absolute and irrevocable right and permission, with respect to my likeness, performance, and participation in its video, photography & audio communications created media.

  1. To record/photograph my likeness, performance and participation;
  2. To copyright the same in its own name or in any other name which it may choose;
  3. To telecast the created media one or more times over any and all manner, in all formats and markets and by any means now known or hereafter devised to publicize the created media or any portion thereof by any means, for any purpose whatsoever in whole or in part, including (but not by way of limitation), promotion, advertising, trade or to reproduce, exhibit, transmit, telecast, broadcast, distribute and otherwise commercially-exploit all or any part of the created media.
  4. To create stock footage and content for future PSAC visual projects.

I acknowledge that Public Service Alliance of Canada, are and will be the sole owner of all rights in and to the created media and the recording/photography thereof, for all purposes in perpetuity. I hereby assign any copyright rights, publicity rights or any other rights that I may have regarding the created media to Public Service Alliance of Canada. I also acknowledge that my likeness may be used in promoting ideas of a political nature and I accept this and hereby release Public Service Alliance of Canada, from any and all claims of any nature whatsoever which I could or might have against the Releases by reason of any fact or matter whatsoever.

Further, I understand that Public Service Alliance of Canada. does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance. I expressly waive any such claim for compensation or liability on the part of Public Service Alliance of Canada beyond what may be offered freely by Public Service Alliance of Canada in the event of injury or medical expenses incurred by me.

This Agreement shall be governed by the laws of Canada. I have read the above document prior to its execution, and I am fully familiar with the contents thereof.
